worked twice then died
Device did manage to work twice. It is now dead (tried three different TVs). Unfortunately it died after return period. Does not work with external power connected.
Packaging did have a hologram.

My Amazon purchase was definitely not a Samsung product. I learned this by purchasing an MHL adapter directly from Samsung. The genuine part is working with my GS4 as advertised. No external power needed to operate but putting power to adapter's power port will charge phone. Automatically connects to HDMI displays.
The difference between the counterfeit and genuine parts are as follow:
Counterfeit: Weighs 18 grams. Hologram on package has poor readability. Date on package back is 2012. The Samsung device label and the MHL plug are rotated 180 degrees. The package's 2D scan code does not link to anything. Cable is longer, 153mm to end of MHL plug.
Genuine: Weighs a hefty 41 grams. Easily readable hologram. Package date is 2014. The Samsung device label and the MHL plug align. 2D scan code links to device's Samsung website. Cable is 143mm to end of MHL plug.
I will attempt to post side-by-side photos of the packaging and devices for the counterfeit and genuine.
